This curse is always the result of a Magical Accident. The character has created a doppleganger. This is an imperfect magical construct from themselves that looks just like them but is their opposite in many ways, a mirror evil twin that will seek to either destroy them, replace them, steal from them all that they are, or cause mischief that gets them blamed for it.

Dopplegangers are made from energies of the seventh dimension, bound with the spirits of fiends, and possessing the memories of the spellcaster that accidentally created them. Their attributes, skills and knowledge match the caster at the moment of creation.
